It's usually the opposite. The first add-on, Hearts of Stone, features an extremely short campaign comparatively. You'll only need to set aside 10 hours on average to finish the main content. Time-conscious players may shave that down to a mere 6 hours, while leisurely witchers can take over 15 hours exploring the additional short story. Hearts of Stone also includes a little bit of its own side content, extending the playtime a bit more. Blood and Wine is the second and far more substantial expansion, acting as the game's epilogue and bringing roughly an additional 15 hours of main story to The Witcher 3 on average.
It also substantially increases the amount of side content by again doubling that runtime if you go for sidequests. If you're like many witcher fans, then you're in this for the long haul and want to know what you're in for taking on the entirety of The Witcher 3 and its two expansions.
Again, that'll depend on how efficient you are at maximizing your time in-game, but here are the estimated time investments required for tackling every conceivable encounter throughout The Witcher 3, Heart of Stone, and Blood and Wine. The Witcher 3 is a sprawling single-player RPG with so much to do it can be overwhelming.
The game world is huge. Points of interest litter the map. Dialogue trees mean you can play through pivotal scenes in different ways. I can think of far worse games to be stuck on a desert island with for the rest of my life.
